Opioid Naive:

Palexia SR 50mg bd

Patients stepping up from weak opioids.

Codeine <360, <400mg- day Palexia 50mg bd If > than 400mg
Palexia 100mg bd

Lower dose if renal or hepatic impairment

Titrate up an extra 50mg bd, can do this every 2 days.
